You can even fax us your forms or drop them off prior to your appointment to speed up your first visit.

Important Appointment Policies
- No-Show Fees
We value your time and ours. Please note that a no-show fee will be applied if you miss your scheduled appointment without prior notice:- First Visit No-Show: $250
- Return Visit No-Show: $185
- Late Arrival Policy
We have a 10-minute late policy. If you arrive more than 10 minutes past your scheduled time, we may need to reschedule your appointment to ensure quality care for all our patients.
New Patient Forms – Please Read Carefully
To streamline your visit and help our providers give you the time and attention you deserve, please do one of the following:
- Option 1: Print and complete all forms ahead of time and bring them with you to your appointment.
- Option 2: Arrive at least 20 minutes early to your appointment to complete the forms in the office.
